A range of standard form contracts and templates are available below for University personnel to download and use.
Each contract has been drafted for a specific type of arrangement and will contain the particular terms and conditions relevant to that type of arrangement. The fact that it is "standard" does not mean it is a generic contract that is suitable for other types of arrangements.
Please refer to the relevant guidelines when using the contracts and templates, and contact the Legal and Risk helpdesk if you require amendments or other support.

Confidentiality agreement (NDA) - (one way)

For use when the University will be disclosing confidential information to an external party before entering into a formal relationship, for example during preliminary discussions and negotiations or when developing a joint submission for a grant application. Also known as a non-disclosure agreement (NDA). Refer to the Confidentiality agreement (one way) guidelines when using this template.

Consultancy services agreement For use when engaging a consultant to undertake straightforward and low-risk consultancy services for a fee of up to $50,000. Refer to the Consultancy services agreement guidelines when using this template.

Credit transfer agreement For use when entering into a formal agreement with a partner institution for a student mobility arrangement which involves granting credit (in a pre-determined and consistent manner) in recognition of studies successfully completed at the partner institution upon admission to certain University programs. Refer to the Credit transfer agreement guidelines when using this template.
Deeds - guidance on signing deeds (University login required) If you manage contracts at the University, you may come across deeds such as deeds of confidentiality, deeds of assignment, deeds of novation, deeds of variation, and deeds of gift. There are formal requirements for signing deeds. As a body corporate, the University is required to sign deeds either under seal or by power of attorney. For more information about deeds, please visit the Deeds webpage (University login required). Deed of Gift To be used to document financial gifts made by donors to the University, including where the gift is made to set up an endowment fund with the University. Electronic signatures guideline Guidelines on approved use of electronic signatures within the University. Executing Written Agreements - Guidance Notes Guidance regarding the different common types of agreements and how those agreements may be signed for and on behalf of the University. Contract approval and sealing request form This form should be used to provide details and context about a contract to the authorised delegate to assist them in understanding what he or she is being asked to approve and refer for sealing. Human research project adverse event report form Use this form to report an adverse event or claim arising from a University Human Research Project or Clinical Trial. Photo / recordings release form For use to obtain a person’s consent for a photo or recording of them to be used by the University. Refer to the Photo / recordings release form guidelines when using this template. If the subject is under 16, use the Photo / recordings release form template for minors. Privacy Impact Assessment (University login required) Use this form to assess privacy risks and recommendations to manage them. Under the University’s Privacy Management Plan a PIA should be completed when an activity or project may involve the collection, storage, use or disclosure of personal information about identifiable individuals.
Request for approval to travel to a high risk destination form (Finance) Destinations designated by smartraveller.gov.au as Level 3 - ‘reconsider your need to travel’ or Level 4 - ‘do not travel’ are considered “high risk” and require additional approval. Prior to making plans to travel to such destinations, University travellers must complete this form and undertake the risk assessment on page 2, attaching any supporting documentation. Request to engage an approved provider form For use when engaging an external legal provider. Refer to Engaging External Lawyers page. In December 2021, Council approved the Legal Engagement (External Lawyers) Policy which sets out the principles and procedures for engaging external lawyers to provide legal services to the University. The Policy ensures that there is appropriate oversight and management of the risks associated with legal issues. Top-up travel insurance application form This form is to request to purchase an extension to cover under the University's corporate travel insurance policy. Refer to the information on the Travel Insurance webpage to find out when you might need to extend your cover.
